Family Practice Residency Grant Program (FPRP)

Intent

The Family Practice Residency Program (FPRP) provides state-funded support to sustain and strengthen accredited family medicine residency programs across Texas. The program operates through an allocation-based funding model that ensures predictable and equitable financial support for all eligible programs.

Funding supports core residency operations, including resident stipends, faculty development, administrative costs, and community-based clinical training. These resources help programs maintain accreditation standards, expand training capacity, and enhance access to primary care in rural and medically underserved communities.

The FPRP advances Texas’ goal of building and retaining a strong primary care workforce through consistent, formula-driven funding. By supporting family medicine residency programs statewide, the FPRP helps prepare physicians to serve communities across Texas, strengthen healthcare delivery systems, and improve access to quality care for all Texans.

The Family Practice Residency Advisory Committee (FPRAC) convened on June 10, 2025, to consider Fiscal Year 2026 funding recommendations, including approval of two new residency programs, Rural and Public Health Rotation awards, and continued support for the Faculty Development Center.

Rural and Public Health Rotation Program

Rural and public health rotations are supported by Family Practice Residency Program (FPRP) funds, as required by a rider in the General Appropriations Act. Both the participating resident and the residency program receive funding under the program; however, rural and public health supervisors serve as volunteers and are not compensated for their services.

Based on Coordinating Board staff recommendations, the Family Practice Residency Advisory Committee (FPRAC) convened on June 10, 2025, and approved 40 rural rotation slots at $2,500 per rotation and three public health rotation slots at $2,000 per rotation for Fiscal Year (FY) 2026. The Coordinating Board approved the funding at its meeting on July 24, 2025.

With the implementation of the new Fluxx Grant Management System (GMS), residency programs will now submit applications for approval on behalf of their residents directly through the GMS system. The FY 2026 Operational Grant Application now contains the embedded Public and Rural Rotation Applications. All applications must be submitted through the GMS system by the posted deadlines. Funding is contingent upon selected residents meeting all eligibility requirements for operational grant funding.

Eligibility

The Family Practice Residency Program (FPRP) provides annual renewable grants to medical schools, licensed hospitals, and nonprofit corporations that sponsor nationally accredited family medicine residency programs. These grants support the educational activities and operational needs of Texas residency programs.

The goal of the FPRP and its associated Public Health and Rural Rotation Program is to increase the number of physicians selecting family medicine as their specialty and to improve access to primary care in rural and underserved communities across Texas.

To be considered for a Family Practice Residency Operational Grant, an applicant must be a medical school, licensed hospital, or nonprofit corporation operating in Texas. Residency programs must be accredited by the Accreditation Council for Graduate Medical Education (ACGME) as a family practice residency program and must have been operational for three or more academic years immediately preceding the application for funds.

Incomplete and late submittals will not be considered for award.

Application Information

Operational Grant:
To be considered for a FPRP Grant, a
pplications must be:

  • Completed in accordance with FPRP published grant opportunity
  • Submit documentation of accreditation by the Accreditation Council on Graduate Medical Education (ACGME)
  • Submitted by an authorized agent of the Applicant entity
  • Submitted via the agency’s grant management system. 

As part of the application process, each participating program is required to submit an electronic roster of residents in training. The roster must include resident demographic information, such as name, training level, and training location, for all certified residents as of September 1, 2025. This data will be used for program monitoring and reporting purposes and will serve as the basis for calculating program allocations under the Family Practice Residency Program.

The Operational Grant Application also includes embedded application components for the optional rotation subgrants. Applicants seeking reimbursement for eligible rotations must complete the following sections within the Operational Grant Application:

  • Public Health Rotation – for residents completing a one-month rotation in a public health setting; and
  • Rural Rotation– for residents completing a one-month rotation in a rural clinical setting.

These embedded applications must be completed at the time of submission to be considered for funding.

A completed resident application, including information on medical licensure or institutional permit, and status and malpractice insurance coverage for any resident choosing to participate in a public health or rural rotation. Following the completion of a qualifying rotation and submission of appropriate documentation, a may be awarded.  

Funding is based on availability. Each resident may be funded for no more than a single one-month rotation per fiscal year (in their first, second, or third year of training, only).  

    • Rural Rotation Program = $1,000 Resident Stipend, and $1,500 Residency Program. 
    • Public Health Program = $500 Resident Stipend, and $1,500 Residency Program.

Rules and Regulations

The Family Practice Residency Program (FPRP) was established in 1977 by the 65th Texas Legislature to strengthen the state’s primary care workforce and improve access to medical services across Texas. The Family Practice Residency Advisory Committee (FPRAC) is charged with making funding recommendations to the Texas Higher Education Coordinating Board (THECB) for the FPRP and related support programs, including the allocation of funds for operational grants and rural and public health rotation programs.

Statutory Authority
The Family Practice Residency Program is authorized under the Texas Education Code, Title 3, Subtitle B, Chapter 61, Subchapter I, Section 61.501, which outlines the Coordinating Board’s authority to administer and oversee grants supporting family practice residency training programs in Texas.

The Family Practice Residency Program (FPRP) advances Texas’ goal of building and retaining a strong primary care workforce through consistent, formula-driven funding. By supporting family medicine residency programs statewide, the program helps prepare physicians to serve communities across Texas, strengthen healthcare delivery systems, and improve access to quality care for all Texans.

Since its inception in 1977, the FPRP has supported approximately 12,300 family medicine residents across Texas, helping to expand training opportunities and strengthen the pipeline of family physicians serving in rural and medically underserved communities throughout the state.
